Cartilage Injury of the Knee

Capitolo:One-Step Chondral and Subchondral Lesion Treatment with MSCs“, pp 289-298.

Articular cartilage is a highly specialized tissue with poor healing potential. Damage to the cartilage following an injury to the joint is prevalent which leads to osteoarthritis. More recent research has focused tissue regeneration and prevention of degeneration. Efforts to repair and restore the hyaline-like cartilage using two-stage procedures such as autologous chondrocyte implantation have led to the development of scaffolds. […]

Sports Injuries

Capitolo: “Capsuloligamentous Knee Corners and Cruciate Ligament Functional Interdependence”, pp 1-15.

The anterior (ACL) and posterior (PCL) cruciate ligaments function in synergy with peripheral capsuloligamentous structures. This chapter discusses how peripheral capsuloligamentous dysfunction can change knee biomechanics, compromising native or reconstructed cruciate
ligament health, potentially leading to reconstruction or repair failure. These functional relationships also influence meniscus and articular cartilage health. […]

Sports Injuries – Prevention, Diagnosis, Treatment and Rehabilitation

Capitolo:Orthobiologic Cartilage Repair and Complex Computer-Assisted Joint Preservation”, pp 1-12.

Articular cartilage injury can be associated with great disability and functional decline. There have been advancements in cartilage repair techniques capable of restoring durable hyaline-like cartilage, even in cases of large or multiple lesions. Single-stage surgical treatments that minimize patient morbidity and provide socioeconomic benefit are ideal. Cartilage repair using a hyaluronic acid-based scaffold combined with bone marrow aspirate containing mesenchymal stem cells (HA-BMAC) has demonstrated successful long-term clinical outcomes for a wide range of patient ages, and when used to treat multiple large articular cartilage defects. […]

Capitolo:Bio-Orthopedics: A New Approach to Osteoarthritis and Joint Disorders“, pp 47-64.

Osteoarthritis is a major cause of functional limitation and a raising burden in aging population. Lately more research is directed into finding biological enhancement of healing processes in joint dysfunctiosn. Biological cell-based therapies for cartilage restoration treatment were created to address the need for the long-term viability of repaired tissues. Additionally, the use of biologic therapies is also considered in common disorders affecting ligaments and cartilage. […]

ESSKA Instructional Course Lecture Book

Capitolo:Anatomy and Function of Articular Cartilage”, pp 43-53.

To properly understand articular cartilage, one must begin with a description of the composition and function of the tissue. Articular or hyaline cartilage is one of the three primary types of cartilage present in the human body. It is the most abundant cartilage, found on the ends of long bones and in synovial joints. Hyaline cartilage is a highly complex tissue organized in four stratified zones consisting of extracellular matrix, non-collagenous proteins, and chondrocytes. […]

The Illustrative Book of Cartilage Repair

Capitolo:The Illustrative Bone Marrow Aspirate Concentrate and Hyaluronan-Based Scaffold Technique for Single-Stage Cartilage Repair“, pp 191-202.

The limited intrinsic healing potential of the articular cartilage is attributed to the presence of specialized cells which have low mitotic activity and lack of blood supply to promote tissue repair. Therefore, once injury occurs, surgical intervention is necessary to maximize the chances of articular cartilage repair. A good cartilage repair will lead to good functional outcome and will avoid subsequent cartilage degeneration that could otherwise lead to the development of osteoarthritis (OA).

Capitolo:The Illustrative Marrow Stimulation Techniques for Cartilage Repair: The Microfracture Technique“, pp 97-103.

Cartilage lesions can occur as a result of an acute injury (i.e., chondral fracture from shear and/or compression forces) or following a repetitive microtrauma during sporting activities. Some studies have shown that the chondral lesions are present in approximately 60% of knee arthroscopies, usually coexisting with the other pathologies. One of the most common coexisting pathology is an anterior cruciate ligament (ACL) tear, where the cartilage lesion acts as a source of a continuing pain and a gradual cartilage degeneration takes place.

Ankle Joint Arthroscopy

Capitolo:Etiology, Classifications, Mechanism of Action“, pp 63-68.

Osteochondral lesions of the ankle are a significant cause of pain and functional impairment. These lesions affect people of all activity levels and may lead to progressive degenerative cartilage injury. Trauma is the most commonly cited etiologic factor. While plain radiography remains an important component of initial assessment, there may be a failure to diagnose osteochondral lesions if additional diagnostic imaging tools are not used. […]

Sports Injuries of the Foot and Ankle

Capitolo:One-Stage Treatment for Osteochondral Lesion of the Talus”, pp 149-158.

Chondral lesions of the talus are usually caused by acute ankle sprain or a chronic ankle instability due to insufficiency of the passive lateral ankle stabilizers. In most of the patients, the lesion remains asymptomatic for a long period of time. Pain, the first clinical symptom of the lesions, usually starts when subchondral bone is affected by the repetitive microtrauma and the chondral lesion becomes an on osteochondral lesion. […]

Capitolo:Tissue Engineering for the Cartilage Repair of the Ankle“, pp 119-124.

Osteochondral lesions of the talus (OLTs) are common cause of postresidual pain after ankle injury. The “gold standard” has not been established over the years, which is why a variety of different methods of cartilage lesion treatment have been used. Microfracture and bone marrow stimulation, although offered fast return to sports, had worse results in a long-term follow-up. Autologous chondrocyte implantation (ACI) was a next step in treatment of OLTs. […]

Return to Play in Football

Capitolo:The Role of Orthobiologics in Return to Play“, pp 273-282.

Bioactive proteins and growth factors are an integral component of healing processes and have the potential to affect metabolic cascades that impact tissue regeneration at all stages. These factors can be used in athletes for a wide variety of musculoskeletal injuries in the form of such biologics as platelet-rich plasma, or in conjunction with cellular therapies, to facilitate return to play. […]

Injuries and Health Problems in Football

Capitolo:Osteochondritis Dissecans of the Knee in Football Players“, pp 189-200.

Osteochondritis dissecans (OCD) of the knee is a frequent cause of pain and functional limitation among skeletally immature and young athletes. Chronic overuse of the lower extremity in young soccer players can cause OCD at the knee and ankle joints; furthermore, with increased competitiveness of amateur sports, more injuries may cause OCD. Increased joint stress, such as an increased pressure at the medial femoral condyle in young patients with genu varum, might cause decreased blood flow and trigger the development of OCD. […]

Revision ACL Reconstruction

Capitolo:How to Handle a Poorly Placed Femoral Tunnel“, pp 87-96.

Majority of primary anterior cruciate ligament reconstruction (ACLR) failures are caused by non-anatomically positioned femoral tunnels. They are predominantly anteriorly-proximally located by trans-tibial tunnel techniques for fear of posterior cortical blowout. For revision ACLR, it is important to avoid overlapping tunnels. […]
